东汉末年，张角手持《风遁天书》，一边治病救人，一边蛊惑民心，妄想改朝换代，黄巾起义各地揭竿而起。起义只是个开始，随之而来的是战火四起，民不聊生。曹操、刘备等忠义之士，以匡扶汉室为己任，欲在乱世建功立业。以两人为首，郭伽、关羽、香香等组成的五人组，结伴而行，一路披荆斩棘，历尽艰险。通过香香的关系，得到了孙坚军队的加盟，最终与张角殊死一战，众人皆陷入张角的幻境中，危难之际，香香选择牺牲自己，破除五感，解救了众人，张角被杀，成功平定了黄巾之乱。
